Opportunity to participate in The Salvation Army's Holiday Angel Tree Program

The Angel Tree program is for low-income families with children ages 12 and younger. Senior Christmas Food Baskets are for Seniors (ages 55+). 

Registration: September 18—21, 2017 

LOCATION: The Salvation Army 3800 9th Avenue North St. Petersburg, FL 33713 

Please bring the following documenta on:

• \uf0a8  Picture ID for all adults in the household (copies are acceptable).

• \uf0a8  Birth Cer cates for all children in the home, 0—12 years of age.

\uf0a8 Child’s immuniza on record with date of birth can be used in lieu of a birth cer cate.

• \uf0a8  Veri ca on of Income (i.e. Pay Stubs, Unemployment, Social Security Bene t State-

  ment, Child Support Statement, Le er of Support, SNAPS, TANF, etc.)

• \uf0a8  Proof of Residency (i.e. U lity bill, Lease) Applicant must live South of Ulmerton Road.

• \uf0a8  If you are not the legal parent, please provide proof of custody of the child.

• \uf0a8  Come prepared with Clothing Sizes (Pant, Shirt, and Shoes) and a Christmas Gi Wish for all children 0—12 years of age.
