Florida Health 2018 Back To School Events

Free vision screening and, for clients who qualify, assistance will be provided for glasses and exam byPreserve Vision Florida on select dates (from 6/5 to 7/24).

Basic dental services to include oral health assessments, oral hygiene instructions, dental sealants, uoridetreatments and referral to a dental home, if needed, at no cost (from 6/5 to 7/24). Medicaid accepted.

Services provided only to students in grades K-12 (not Pre-K).
Parents must bring the child’s immunization record and be present for services. If the parent is not present, a verbal consent must be obtained prior to receiving services. Services provided at the following locations:

PINELLAS PARK HIGH SCHOOL CLINIC – 6305 118th Ave N. in Largo Monday-Thursday from 6/5 to 6/29 and 7/24 to 8/8 (except 7/26) from 8:00 AM to 2:45 PM. (Come to the front entrance of the school and the clinic is to the left with a separate entrance.)

NORTHEAST HIGH SCHOOL CLINIC – 5500 16th St. N. in St. Petersburg Monday-Thursday from 6/5 to 6/29 and 7/24 to 8/8 from 8:00 AM to 2:45 PM.
(After entering front entrance, go almost to the end of the parking lot and the clinic is on your right.)

GIBBS HIGH SCHOOL CLINIC – 850 34th St. S. in St. Petersburg
Monday-Thursday from 7/9 to 8/8 from 8:00 AM to 2:45 PM. (Come through the main entrance, make a rightand make the rst left; the clinic is on your left with a separate entrance.)

BOCA CIEGA HIGH SCHOOL CLINIC – 924 58th St. S. in Gulfport
Monday-Thursday from 7/24 to 8/8 from 8:00 AM to 2:45 PM. (Park at the main entrance, enter through front doors. At the front desk, tell them you have an appointment at the clinic.)
